• Live-in care, waking nights/sleep-ins, respite and reablement after hospital discharge.
• Co-ordination with families, Local Authorities, and NHS partners.

• Older adults and adults with physical disabilities.
• People living with complex health needs or long-term conditions.
• Individuals requiring palliative/end-of-life support.
